Awe-Inspiring Consultants has been Contracted, Credentialed, and Approved as a provider for HCBS-AMH services with the Health and Human Services Commission since May 2016. Adults with a diagnosis of Serious Mental Illness (SMI) and those interested in receiving HCBS-AMH services with Awe-Inspiring Consultants must have: Medicaid, SSI, and a history of at least one of the following additional criteria:

STEP 1: Supervised Living Group Homes for Individuals with Mental Illness

• You must have active Medicaid
• Age 21 or older
• History of mental illness/Substance use disorders
• Must bring ALL copies of (each) hospital admission as proof

A. Four arrests and Two Psychiatric Hospitalizations within the last three years.

B. Fifteen Emergency Room and two psychiatric hospitalizations within the last three years.

(OR)

C. Spent 1095 days or more within a psychiatric hospital in the last five years.
